SPORTS PHYSICALS
In partnership with Kelly Care, Boosters offers affordable sports physicals to current River Hill students and incoming freshmen. Howard County Public School System policy requires every athlete to have verification of a physical exam dated after Aug 23, 2024 in order to try out for any high school sport.
Kelly Care generously donates all proceeds from the Sports Physicals program back to River Hill Boosters.
IMPORTANT: This sports physical should not replace the annual visit with the athlete's primary care physician.
HOW MUCH: $30. In order to receive the $30 sports physical, you must pre-purchase via the link below.
WHERE: All sports physicals take place at Clarksville location of Kelly Care, located next to the River Hill Gym and Giant.

- Purchase using the link below.
- Call to easily make an appointment. 888-528-8321
- Or, head to the Kelly Care website and secure your appointment via their online portal.
- When making the appointment choose Kelly Care as your location. This is the Clarksville location. Do not choose Kelly Goodman, NP & Associates, PC.
- When they ask for APPOINTMENT TYPE, choose RIVER HILL SPORTS PHYSICAL. This is located at the bottom of the list of appointment types.
- When you are asked about insurance, choose NO INSURANCE. Regardless if you have your own insurance, please register without it.
- Complete the required online booking questions and confirm your appointment.

- Your email confirmation is your receipt. You must print the email and bring with you as your proof of payment.
- Pre-Participation Physical Evaluation Form. This form is to be filled out by the patient and parent prior to seeing the physician. Click here to access and print the evaluation form.

Physicals are currently walk in only (you do not need to make an appointment). First Call Urgent Care is open Monday-Friday (8am-8pm), Saturday-Sunday (8am-5pm).
